Birth, Marriage and Death Certificates
Requirements to obtain a Birth, Adoption, Marriage or Death Certificate from the Cooperative Republic of Guyana are as follows:

Download appropriate application forms from our website or obtain forms free of charge from the Consulate.

Copy of Birth, Marriage or Death Certificate if available

Submission of Deed Poll if applicable

Submission of appropriate completed form and copy of applicable certificate if available.

Fee – $10.00 CND cash or money order made payable to the Consulate General of Guyana
Guidelines for Completing the Forms
Applicants must ensure that:

All relevant sections of the applicable form is completed

The registered name of the applicant is inserted on the applicable form (call name will not be accepted)

All supporting documents are attached

Applications will be subjected to delays if information inserted on the Form is not accurate.
Procedure

Completed applications are forwarded to the General Registrar Office for processing which takes approximately two (2) to three (3) months.

Upon receipt of the Certificate, the applicant will be contacted via telephone to uplift their Certificate from the Consulate

For applicants who cannot appear in person, an authorization letter will be required.
