Renunciation of Guyanese Citizenship
Persons eligible to apply for a Machine-Readable Passport (MRP):
- Persons of Guyanese citizenship by birth, descent, registration and naturalization
- Guyanese Nationals whose Passports are lost, stolen or damaged
- Guyanese Nationals whose passport are expired
- Guyanese Nationals whose passport has less than six (6) months validity for international travel
N.B. Applicants who are in possession of expired passports are urged to apply for the new Machine Readable Passport (MRP) as soon as possible.
General Information–Applying for a new (MRP)
- Application Forms (Form A) can be downloaded from the Consulate website or uplifted free of cost from the Consulate.
- Applicants are encouraged to read instructions on page 1 of the Form before completing application.
- Completed Applications are sent to Georgetown for processing which takes approximately six (6) to eight (8) weeks
- The Consulate does not retain passports. The appropriate pages are copied and returned to the applicant
- Before the new passport is issued to the applicant, it is mandatory that the applicant produce the old passport for cancellation
- Applicants who are unable to produce their old passport will be required to produce a Police Report detailing the circumstances under which the old passport was lost/misplaced, before the new passport is issued.
- Passports are valid for five (5) years and are not renewable.
- Fee – $95 CND for New Passports and $195 CND for replacement of
Lost and Damaged passports (Cash or money order made
payable to the Consulate General of Guyana).
- Applications will be subject to delays if all requirements are not met.
Guidelines for Completing the Passport Application Form
Applicants must ensure that:
- The Application Form is filled out correctly and submitted with all relevant supporting original documents.
- The name that the applicant wishes to be recorded in the new passport corresponds with that on the application form and supporting documents
- Applicant’s matching signatures are affixed within the designated rectangular box on page 2 and the declaration section on page 3.
- A suitable recommender completes Section 7 of the Form and endorses the reverse side of one passport sized photograph. (Only persons who are not in possession of Machine Readable Passports and those who have lost their passports are required to complete Section 7)
- The Recommender endorses the reverse side of one (1) photograph with the words: “I certify that this is a true likeness of …” and adds his or her signature.
- The Recommender affixes his/her Official Stamp/Business Card and phone number on the application form
- Passport-sized photographs have a white background and are not larger than 35 x 45 mm nor smaller than 26 x 32 mm

Holders of the old Green Guyana Passport
- Completed Guyana Passport Application Form.
- Signature card (obtained at the Consulate at the time of submission of application)
- Original Birth, Registration or Naturalization Certificate where applicable
- Old Green Guyana Passport
- Two Passport sized photographs endorsed and stamped.
- Deed Poll if applicable
Holders of Machine Readable Passports

Married Women who wish to change their Surname
- Completed Guyana Passport Application Form

- Current or expired Guyana passport
- Original Birth, Registration or Naturalization Certificate where applicable
- Husband’s Birth Certificate
- Marriage Certificate/Name Change Certificate
- Deed Poll if applicable
- Two Passports sized photographs.
Applicants under 16 years
- Completed Guyana Passport Application Form
- Signature card (obtained at the Consulate at the time of submission of application)
- Original Birth, Registration or Naturalization Certificate/Adoption Certificate where applicable
- Current Passport, Adoption Certificate if applicable
- Valid Identification document(s) of parent (s) or guardians
- Two Passport sized photographs
- Deed Poll if applicable
- Applicants under sixteen (16) years must be accompanied by one or both parents. In cases where either parent or guardian is unavailable, a sworn Affidavit from a Notary Public is required. The affidavit must indicate that the child/children is/are in the custody of either parent making the representation to apply and uplift the passport for the child/children and there is no objection to the application for a new passport by the minor.
- Parents/guardians are required to sign the appropriate section on page 3
Applicants in possession of Damaged Passport
- Completed Guyana Passport Application Form
- Signature card (obtained at the Consulate at the time of submission of application)
- Damaged Passport
- Affidavit in support of damaged Passport
- Original Birth, Registration or Naturalization Certificate where applicable
- Two Passport sized photographs stamp and endorsed
- Deed Poll if applicable
Applicants whose Passports were Lost, Stolen or Destroyed by Fire
- Completed Guyana Passport Application Form
- Signature card (obtained at the Consulate at the time of submission of application)
- Original Birth, Registration or Naturalization Certificate where applicable
- Police Report/Fire Report (where applicable), and Affidavit in Support of Lost Passport
- Two Passport sized photographs
- Married women whose surname was changed by marriage would be required to produce their husband’s birth certificate and Marriage certificate/Name Change Certificate
- Divorce absolute, deed Pool and registration or naturalization certificate where applicable
Submission of Guyana Passport applications by mail in service
To accommodate persons residing out of the Greater Toronto Area (GTA) the Consulate offers a Mail in Service to accommodate this category of persons applying for new passports.
Persons who are desirous of utilizing this service are required to:
- Download Passport Application Form
- Read the instructions on page one of the application form and complete all relevant sections. Please refer to Guidelines for Completing the Passport Application Form and Checklist for documents that are required to be submitted for the various categories of applicants
- Before submitting the passport applications, applicants are required to book a virtual interview by calling telephone numbers 416-494-6040 or 416-494-6059 between 9 am – 4 pm Monday to Friday or via email address info@guyanaconsulate.com.
- Applicants will be required to submit their passports and original supporting documents
- Make out a money order payable to Consulate General of Guyana( $95 CND for a new passport and $195 CND for the replacement of a lost/damaged passport as applicable)
- Applicants in Ontario are required to submit application and all supporting documents via Regional Express Post or courier services such as FedEx, DHL, UPS, with return courier service envelope
- Applicants who are outside of Ontario are required to submit application and all supporting documents via National Express Post or courier services such as FedEx, DHL, UPS with return courier service envelope
Procedure
- Upon receipt of the Completed Passport Application Form, the Consulate will ensure that all relevant sections of the Application Form is completed and matches the information on the original supporting documents.
- Once the application meets the requirement, same will be forwarded to Guyana for processing.
- Upon receipt of the new passport from Guyana, the old passport will be cancelled and returned along with all original supporting documents and the new passport via return mail.
N.B Processing time is six (6) to eight (8) weeks
Requirements for Passport sized Photographs
All applicants must ensure that both identical passport size photographs meet the following specifications:
- Applicant must be fully covered/properly fully clad.
- Dimensions of the photograph meet the requirements as stated on page one of the Application Form (minimum 26x32mm, maximum 35x45mm).
- Clarity and contrast are correct, proper exposure (no washout).
- No shadows or glare from improper lighting, glasses or jewellery.
- Background is plain white (no colour or shadows).
- Neutral expression (No smile, open mouth or frown).
- No head piece or hair covering (except those worn for religious purposes & only if the full facial features are clearly visible).
- No sunglasses.
- Eyes, Ears, Full Facial Features are clearly visible.
- Studs/Earring/s not accepted on male applicants pictures.
- Photos are stamped with photographer stamp and dated
- Stamp and date must be legible (not smeared).
- No glare/shine.
- The reverse side of one photograph is endorsed by the recommender with the words: “I certify that this is a true likeness of …” and his or her signature is affixed (only applicable for persons who are applying for the replacement of a lost/damaged passport).
N.B. Photographs that do not meet the above mention specifications will not be accepted.
Acceptable Recommenders
While page 1 of the Passport Application Form states that the recommender should be Guyanese, this requirement is not applicable for Guyanese who are resident overseas. However the recommender must be professionally known to the applicant for over two (2) years.

- Doctor/Dentist/Registered Nurse (Reg.#)
- Imam/Pandit/Priest (with Lic# or Business Card/Office Stamp)
- Lawyer/Notary Public/Justice of Peace
- Pharmacist (with Lic#)
- Professional Engineer (with Lic# or Business Card)
- Police Officer (with Badge#)
- School Principal (with Office Stamp/Card)
- University/College Professor(with Office Stamp/Card)
